Commence Your Healing Journey

To initiate your journey towards improved mental health and wellness, please complete the provided form. Our appointments are scheduled once this form is submitted, enabling us to create a patient record within our electronic health system. We kindly request copies of your insurance card (front and back) and a copy of your license to verify your identity. You can effortlessly upload these documents via this form or send them to admissions@smhwi.com. Following the completion of these steps, one of our admissions representatives will contact you to arrange an appointment.
